protocol__form__id-formnumber.yaml

slashes (directory separators) get converted to underscores.

For example, given the first form at /example/inputscreen defined in protocol exprot,

exprot__form__example_inputscreen-0.yaml will define the layout within the template api.

Good practice is to define the expected incoming variables within /_meta/vars
